About The Position

We are seeking dedicated Pediatric Clinical Liaisons to join our team and help create impactful patient experiences. In this role, the Pediatric Clinical Liaison serves as a key contact for hospice and palliative care services, working directly with assigned accounts to support admissions, provide education, and assist with discharge planning. The liaison will educate patients and families to ensure appropriate admissions, oversee pending referrals, and track the progress of prospective patients. Collaborating closely with patient care teams, the Pediatric Clinical Liaison will facilitate smooth transitions as patients are admitted to services. They will also initiate the admission process by informing and educating families about available services. Additionally, the liaison may provide consultation on nursing assessments, initial care plans, and interventions to support effective discharge planning. The Pediatric Clinical Liaison will assist in patient and family informational meetings for both Hospice and Palliative Care during the referral process when necessary. This position is designed to drive growth in admissions, foster strong professional relationships with key physicians and hospital staff, and support the Pediatric Hospice and Palliative Care team in meeting growth and development goals. The role will also focus on building market trust and loyalty. Requirements

  • Graduate of an accredited school of nursing.
  • Registered nurse currently licensed in the state(s).
  • Certified Hospice and Palliative Nurse (CHPN) preferred.
  • Must successfully perform and comply with all credentialing as required by hospital/health care facility.
  • Minimum of 3-5 years general nursing required.
  • Minimum of 2-3 years of Pediatric nursing required.
  • Hospice and or Palliative Care experience preferred.
  • Knowledge of clinical manifestations of diseases and ability to develop home care/hospice-specific clinical knowledge.
  • Understanding of hospice philosophy and the principles of death and dying.
  • Ability to market to and deal tactfully with customers, referral sources and the community.
  • Excellent communication, negotiation and public relations skills.
  • Excellent presentation and public speaking skills.
  • Compliance with accepted professional standards and practices.
  • Reliable transportation.
  • Ability to sit, stand, bend, move intermittently and lift at least 80-100lbs and bear the weight of an average adult effectively.

Responsibilities

  • Acts as liaison for hospice and palliative care.
  • Actively calls on assigned accounts to directly assist with admission development, education and discharge planning.
  • Initiates the admission process by educating and informing patient/family of services.
  • Consults as appropriate on nursing assessment, initial plan of care and intervention(s), to optimize discharge planning.
  • Supports / facilitates patient/ family informational meetings for all both Hospice and Palliative Care during referral process as necessary.
  • Promotes growth of admissions over time.
  • Establishes and maintains significant professional relationships with key physicians and hospital personnel.
  • Works closely with Pediatric Hospice and Palliative Care team to meet growth and development targets and actively establish and maintain market acceptance and allegiance.
  • Explains services to patients and families in a manner that results in the admission of appropriate patients.
  • Interacts with patients, families, and referral sources.
  • Oversees pending patients (referrals) and routinely follows the progress of prospective patients.
  • Works closely with patient care teams in transitioning patients as they are admitted to services.
  • Receives, interprets and manages patient/ client referrals with the intent of converting to actual admissions for HCSC services.
  • Provides community and provider in-service education in assigned territory.
  • Maintains daily communication with Customer Relations Department and Team Directors.
  • Works with the Hospice Liaison to develop and implement individualized marketing plan.
  • Maintains visibility and accountability to increase relationships with contracted inpatient facilities and physician groups.
  • Acts as a liaison with Hospice Medical Director, Palliative Care RN, CHC and Hospice Clinical Teams, Team Physicians and Hospice Physicians on behalf of providers and patients/families.
  • Makes inpatient visits at contracted facilities and communicates with Customer Relations Department, Directors and/or Primary Nurse.
  • Provides consultation on hospice, home health and palliative care.
  • Demonstrates familiarity with policies of the agency and state(s) and federal regulations.
  • Participates on committees and special projects and other related duties as assigned.
  • Performs evaluations, sign ups, assessments, and discharge planning of new patients primarily in contracted hospitals.
  • Formulates the initial interdisciplinary plan of care and communicates to the Team Director.
  • Obtains data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.
  • Maintains confidentiality of sensitive information.
  • Participates in hospital orientation and ongoing education programs.
  • Attends outside seminars or educational opportunities to increase knowledge and skills.
  • All other duties as assigned.

  • Continues professional growth through attendance at workshops, professional in-services and through individual research and reading.
  • Written and verbal communication is expressed clearly and in a cooperative manner.

  • Detail oriented with good time management skills.
  • Serves on agency committees and participates in problem-solving and ways to improve systems in patient care.
  • Monitors systems and identifies problem areas through written reports.
  • Reports and follows-up on identified patient concerns and quality of care issues to the appropriate individuals.
  • Maintains on-going statistics of patient liaison activity and referral sources.
  • Above average documentation skills.

  • Provides positive, supportive communication to physicians, patients, families, visitors, and other agency personnel.
