About The Position

Angels of Care Pediatric Home Health is seeking compassionate and dedicated Registered Nurses (RN) and Licensed Vocational Nurses (LVN) to provide direct, one-on-one medical care to medically fragile infants, children, and adolescents in their homes. This role focuses on delivering patient-centered care according to physician orders and active plans of care, ensuring positive patient and family outcomes through ongoing assessments, medication administration, treatment, therapy, care coordination, and family education. Requirements

Active RN or LPN/LVN license (New Grads Welcome, training provided!)
Provide care in a client home setting

Responsibilities

Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
Conduct on-going patient care and assessments.
Administration of prescribed medication, treatments, and therapies.
Coordination of care.
Educate family members on patient clinical care to enhance positive outcomes.
Preventative initiatives to protect quality of care for patient.
