About The Position

Angels of Care Home Health is seeking compassionate and dedicated Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care to medically fragile infants, children, and adolescents in their homes. This role offers full-time and part-time shifts, including days, nights, and weekends, with a focus on patient-centered care and making a significant difference in the lives of patients and their families. Requirements

• Active RN or LPN/LVN license
• Ability to provide care in a client home setting
• Ability to make a positive and lasting impression

Responsibilities

• Provide direct, one-on-one medical care to infants, children, and/or adolescents primarily in their home according to the active plan of care.
• Conduct on-going patient care and assessments.
• Administer prescribed medication, treatments, and therapies.
• Coordinate care.
• Educate family members on patient clinical care to enhance positive outcomes.
• Implement preventative initiatives to protect the quality of care for patients.
