About The Position

RiverStone Health is seeking a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) to join their Behavioral Health team. This role focuses on assessing and treating individuals with mental, emotional, or substance abuse issues, primarily within a school setting or the main RiverStone Health Clinic. The position involves individual and/or group therapy, crisis intervention, case management, client advocacy, prevention, and education. The provider will collaborate with physicians, school staff, and other clinic staff to deliver solution-focused care and connect patients and clients with appropriate social services to address their needs. This role operates within a Patient Centered Medical Home model of care.

Requirements

  • Master’s degree in psychology, counseling, social work or related behavioral health field.
  • Any combination of experience and training which provide the equivalent scope of knowledge, skills, and abilities necessary to perform the work.
  • Experience providing pediatric and adult mental health care
  • Valid State of Montana driver's license
  • Current Montana LCSW or LCPC license
  • BLS Certificate
  • Ability to maintain a calm and positive demeanor during difficult client interactions.
  • Ability to display non-judgmental and empathetic listening skills.
  • High degree of detail-oriented skill level.
  • Knowledge and understanding of protected sensitive patient health information (HIPAA) and confidentiality.
  • Ability to work collaboratively and maintain a positive work environment.
  • Ability to communicate clearly and accurately with supervisors, other RiverStone Health employees, community resources and clients and patients.
  • Ability to perform job duties with integrity and innovation to ensure completion and a high level of quality.
  • Ability to understand and adhere to required administrative policies and procedures.

Responsibilities

  • Counsel clients in individual or group sessions to assist them in dealing with substance abuse, mental or physical illness, poverty, unemployment, physical abuse, etc.
  • Collaborate with counselors, physicians, school staff, or nurses to plan or coordinate treatment, drawing on social work experience and patient needs.
  • Monitor, evaluate, and record client progress with respect to treatment goals.
  • Interview clients, review records, conduct assessments, or confer with other professionals to evaluate the mental or physical condition of clients or patients.
  • Modify treatment plans according to changes in client status.
  • Assist clients in adhering to treatment plans, such as setting up appointments, arranging for transportation to appointments, or providing support.
  • Educate student and staff members about mental or physical illness, abuse, medication, or available community resources.
  • Counsel or aid school staff members to assist them in understanding, dealing with, or supporting the client or patient.
  • Increase social work knowledge by reviewing current literature, conducting social research, or attending seminars, training workshops, or classes.
  • Refer patient or client to community resources for housing or treatment to assist in recovery from mental or physical illness, following through to ensure service efficacy.
  • Represent RiverStone Health in meetings, trainings or public events when needed.
  • Provide triage via telemedicine or in-person as needed.
  • Attend essential clinic and all staff meetings.
  • Attend continuing education trainings to maintain licensure.
  • Prepare and give presentations during clinic, all staff, and other meetings within the organization.
  • Potential requests to do community presentations.
  • Involvement in media publications.
