Pediatric Behavioral Health Care Manager (BHCM)

Fort Health
$35,000 - $40,000Hybrid

About The Position

This is not a traditional therapy role. As a Behavioral Health Care Manager, you will be the core member of an integrated, multi-disciplinary treatment team supporting a panel of patients with pediatric mental health needs. You’ll practice in a true Collaborative Care Model (CoCM), working alongside pediatricians and a psychiatric consultant to deliver measurement-based, outcomes-driven care to children and families at a population health impact. If you value wrap-around, evidence-based care, clear clinical direction, and a highly collaborative team environment, this role is designed for you. Fort Health is building a better system for pediatric mental health—one that is accessible, evidence-based, and integrated into primary care. We partner with leading organizations like the Child Mind Institute and follow the AIMS Center model to deliver care that is both scalable and clinically rigorous.
