The Behavioral Health Care Manager plays a key role in delivering integrated mental health services within the Collaborative Care Program. This position is responsible for conducting mental health screenings and assessments, as well as providing evidence-based treatments for conditions such as depression, anxiety, and traumatic stress in pediatric patients with chronic illness. The Behavioral Health Care Manager educates patients on managing their conditions, monitors clinical outcomes, and tracks symptom progression or complications. Utilizing therapeutic approaches such as Cognitive Behavioral Therapy (CBT) and Motivational Interviewing, the Behavioral Health Care Manager delivers brief, targeted interventions. This role works closely with nephrology providers, psychiatric consultants, and caregivers to develop and implement individualized treatment plans. Additional responsibilities include care coordination, advocacy, case reviews, and facilitating transitions across levels of care.
