About The Position

The role of the PCMH Clinician Lead is to provide support to the practice in the implementation of the Patient Care Medical Home (PCMH) model. The PCMH Clinician Lead sets the tone for how the practice will function as a medical home. The PCMH Clinician Lead is an integral part of the PCMH team.

Requirements

  • Associate degree required
  • Excellent organizational skills
  • Excellent communication skills
  • Excellent time management skills
  • Knowledge of computers and EHR
  • Knowledge of the practice’s clinical operations
  • Minimum of 2–3 years of experience in healthcare, primary care, pediatrics, ambulatory care, care coordination, quality improvement, or clinical operations.

Nice To Haves

  • bachelor’s degree preferred in healthcare administration, public health, nursing, social work, business, or related field.
  • Knowledge of PCMH model, NCQA standards, practice workflows, EHR documentation, and quality metric tracking preferred.

Responsibilities

  • Assist and guide staff with roles within the PCMH model
  • Promote and develop staff education related to areas of PCMH improvement.
  • Facilitate teamwork between physicians, specialists, clinical teams and patients/parents
  • Facilitate communication among staff related to PCMH activities
  • Responsibilities for Quality Improvement Leadership
  • Collaborates with IT/vendor for EHR Customization
  • Contributes to practise policy and procedure development.
  • Other duties as assigned by supervisor.
