About The Position

The primary purpose of your job position is to provide each of your assigned residents with routine daily care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. This role involves creating and maintaining a warm, positive, and calm environment for residents, assisting with daily living activities, and ensuring resident comfort and safety. The PCA will also be responsible for accurate record-keeping and reporting of any changes in resident condition or incidents.

Requirements

  • Must possess, as a minimum, a high school education or its equivalent.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Proficient in Microsoft Suite products.

Responsibilities

  • Provide routine daily care and services to assigned residents according to their assessment and care plan.
  • Use PCC to ensure correct resident information when assisting with ALDS, meals, etc.
  • Report all changes in residents’ condition to the Health and Wellness Director promptly.
  • Record all entries on notes, SPAs, and incident reports in an informative and descriptive manner.
  • Report all observed accidents and incidents to the Executive Director and Health and Wellness Director.
  • Maintain resident confidentiality and report any suspected violations.
  • Report any known or suspected unauthorized attempts to access the community’s information system.
  • Perform only trained care tasks.
  • Ensure resident rooms are prepared and comfortable.
  • Greet residents and escort them to their rooms.
  • Introduce residents to others and make them feel welcome.
  • Inventory, mark, and store residents’ personal possessions and clothing.
  • Assist residents with packing for transfers or discharges.
  • Transport residents to new rooms or receiving areas.
  • Assist with loading/unloading residents to/from vehicles.
  • Create and maintain a warm, personal, positive, and calm atmosphere.
  • Meet with nursing personnel to identify and correct problem areas.
  • Report all resident complaints and grievances.
  • Participate in and receive nursing reports.
  • Follow established policies concerning exposure to blood/body fluids.
  • Make beds and add extra covers as requested.
  • Ensure residents who are unable to call for help are checked frequently.
  • Answer resident calls promptly.
  • Check residents routinely to ensure personal care needs are met.
  • Assist residents with identifying food arrangements.
  • Keep resident water pitchers clean and filled.
  • Perform after-meal care.
  • Receive the nursing report upon reporting for duty.
  • Follow established policies, procedures, and supervisor instructions.
  • Follow work assignments and schedules.
  • Cooperate with inter-departmental and facility personnel.
  • Notify the facility when late or absent.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Assist residents with daily functions (dental care, bathing, hair combing, dressing/undressing).
  • Keep residents dry by changing clothing and linens when wet or soiled.
  • Assist residents in preparing for medical tests.
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Perform restorative and rehabilitative procedures as instructed.
  • Assist in preparing residents for physical examinations.
  • Observe and report pressure areas and skin breakdowns.
  • Report injuries of unknown source.
  • Observe disoriented and comatose residents.
  • Record and report data as instructed.
  • Perform special treatments as instructed.
  • Immediately notify the Nurse of any resident leaving/missing from the facility.
  • Use only trained equipment.
  • Report defective equipment to the Nurse Supervisor/Charge Nurse.
  • Maintain confidentiality of resident care information.
  • Report known or suspected incidents of unauthorized disclosure of information.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor resident refusal of treatment requests and report them to a supervisor.
  • Give back rubs as instructed.
  • Assist residents with hair care functions.
  • Assist residents with nail care.
  • Shave male residents.
  • Keep hair on female residents clean as instructed.
  • Assist residents with bowel and bladder functions.
  • Maintain intake and output records.
  • Keep incontinent residents clean and dry.
  • Check and report bowel movements and character of stool.
  • Utilize protective and adaptive equipment as instructed.
  • Provide eye and ear care.
  • Weigh and measure residents as instructed.
  • Measure and record temperatures, pulse, and respirations (TPRs).
  • Assist with the care of the end-of-life resident.
  • Assist with post-mortem care as instructed.
