About The Position

The Patient Services Coordinator plays a crucial role in delivering healthcare services and serves as a liaison for patient care activities in the practice. The role helps ensure quality patient care during visits, educates patients about their care plans, and coordinates services with Primary Care Providers and outpatient diagnostic testing facilities.

Requirements

• Some college education.
• Three years of healthcare, customer service, or relevant experience.

Responsibilities

• Work cohesively with the Primary Care Provider to facilitate patients’ care plans by participating in daily huddles and monthly case discussions, facilitating appointments, and coordinating information and continuing-care requirements.
• Collaborate with other clinical staff to resolve issues affecting patients’ care progression.
• Monitor and update the dashboard with reminders and alerts to organize daily tasks, including visit-status reminders, follow-up appointments, authorizations for medical review, specialty appointment coordination, and billing-error resolution when applicable.
• Contact insurance carriers for patient pre-certification and consult with the Department/Practice Manager about patient needs or escalated issues.
• Provide front-end coverage as needed and record telephone encounters for routing to appropriate Providers.
• Act as a liaison among Physicians, patients, and clinical staff to connect patients with relevant internal and external resources.
• Recommend process and service improvements to promote patient satisfaction and address issues.
• Perform other duties as assigned.
