Improve the overall health of the communities we serve by enrolling, educating patients about CHAS services and community resources, and overseeing the management of patients assigned to or eligible for CHAS managed care contracts. This role involves working with a diverse patient population, determining eligibility for various health insurance and assistance programs, and enrolling patients in Medicaid, CHIP, Qualified Health plans, and Qualified Dental plans. The coordinator will also educate patients on their insurance plans, assist with dental treatment plan reviews, and inform new patients about CHAS Health services. Additionally, the position requires participation in community outreach programs, providing training to staff, and tracking enrollments and managed care conversions.
