About The Position
The Patient Placement Coordinator RN in Anaheim, CA, is a Full-time position working 10-hour, Variable shifts. This role supports care for Providence St Joseph Health (PSJH) patients by evaluating and coordinating the patient intake process. Responsibilities include taking and triaging calls, and facilitating inbound and outbound transfers for PSJH facilities. The coordinator acts as a liaison between physicians, hospitals, and other care providers for patient referrals, admissions, and transfers, ensuring timely medical communication. Critical thinking is employed to integrate objective data with preliminary information and ongoing assessments, including patient experiences and health care history, for optimal placement. The RN also serves as a clinical resource, providing guidance on clinical concerns, level of care determination, and regulatory considerations. Additionally, they support the onboarding and training of new staff to reinforce consistent practices.
Requirements
- California Registered Nurse License upon hire.
Nice To Haves
- Bachelor’s Degree from an accredited School of Nursing.
- Bachelor's Degree in Nursing.
- Minimum of 2-5 years nursing experience.
- Working knowledge of Epic and Bed management system.
