Patient Nurse Navigator - Weight Management Center
About The Position
The Patient Nurse Navigator role is within the Weight Management Center, specifically at the Winston-Salem University Group Practice. This is a full-time position, Monday-Friday, 8a-5p, with no weekends, nights, or on-call duties. The position is primarily based at the Country Club location in Winston-Salem but may require support at Greensboro or High Point locations based on coverage needs, with specifics negotiable. This role operates in an ambulatory, multi-disciplinary clinic setting, collaborating with physicians, APPs, Exercise Physiologists, Registered Dietitians, and Behavioral Health providers to help patients achieve their care goals. The position also supports virtual patient visits and offers opportunities for team collaboration, ongoing education, and professional development. The core responsibilities include patient tracking, in-basket management, coaching visits, and patient education, engaging in dynamic care related to obesity management. The role will support both medical and surgical weight management patients, as well as general surgery patients seen at the location for consults and post-op appointments. No procedures or complex care are provided at this ambulatory site.
Requirements
- Graduate from an accredited school of nursing
- BSN required
- Minimum two years' experience as a registered nurse in the specialty required
- Current licensure to practice as Registered Nurse in applicable state
- BCLS required
- Requires problem solving, decision making and critical thinking
- Ability to work in a self-directed environment and with leading teams
- Demonstrates ability to work in fast-paced environment with multiple interruptions
- Expertise in MS Office
- Ability to prepare program reports frequently and efficiently
Nice To Haves
- Certification as an RN preferred
- Bilingual preference with fluency in Spanish desirable
Responsibilities
- Utilizes the nursing process to provide patient/family centered care focusing on the physical, emotional, spiritual, cultural, religious, and environmental needs.
- Coordinates the effective and efficient delivery of patient care services through the application of care management principles.
- Documents in accordance with departmental reporting standards.
- Collects and maintains up to date and accurate program data and documentation according to program standards.
- Delivers reports that demonstrate outcomes and performance improvement activities; adheres to established productivity benchmarks.
- Implements outreach efforts to establish and maintain positive working relationships with key community agencies, serves as community liaison and educator in underserved communities.
- Demonstrates the ability to function in a professional setting through active participation in a professional practice model.
- Communicates effectively to promote sharing of information throughout the continuum of care and as an effective member of the healthcare team.
- Responsible for ongoing evaluation and quality improvement of the Navigator Program and will track all activity as defined by the departmental and program standards.
- Works closely with community-based service providers to identify and address community needs for services and education regarding prevention and/or early detection, medications and first aid available.
