About The Position

Ascension Via Christi, Rehabilitation Hospital is seeking a Registered Nurse Patient Navigator to join their Administration department. This is a full-time, day shift position. Requirements

• Licensed Registered Nurse credentialed from the Kansas Board of Nursing required.
• Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.

Nice To Haves

• BLS Provider preferred. American Heart Association or American Red Cross accepted.

Responsibilities

• Serve as a dedicated patient navigator throughout the continuum of care, ensuring individuals experience seamless, well-coordinated transitions and feel supported at every step of their health journey.
• Function as a central liaison between management, physicians, managed care companies, and community organizations to streamline communication and deliver a unified care experience.
• Coordinate strategic treatment efforts while actively promoting quality improvement initiatives and educational programs that elevate overall standards of care.
• Compile and analyze key clinical data and outcome tracking to drive meaningful process improvements across the department.
• Provide vital support and resource referrals for uninsured or underinsured patients, bridging the gap to ensure everyone has access to essential follow-up care.
