About The Position

The Patient Nurse Navigator will coordinate and conduct patient assessments utilizing community paramedics for community, 9-1-1, and third-party referrals, as well as qualifying inpatient discharges prior to hospital release. This role involves collaborating with hospitals and facility staff to support on-site discharge planning and care transitions. The navigator will work closely with physicians, PA-C’s, RNs, behavioral health providers, case management, pharmacy, and insurance plans to ensure continuity of care. Responsibilities include performing SDOH and inpatient assessments to identify clinical, behavioral, social, and environmental needs, and developing, implementing, and monitoring individualized care and aftercare plans. The position facilitates referrals to various community and governmental agencies, coordinates resource management, and provides patient and family education. The navigator will implement and support appropriate clinical interventions delivered by community paramedics, manage complex patient cases including crisis response, and develop targeted outreach strategies for patients lacking consistent healthcare engagement. Additionally, the role involves identifying barriers to care, collaborating on integrated plans of care, participating in referral conferences and interagency meetings, and ensuring accurate case management and documentation. The navigator will also identify system and provider issues and contribute recommendations for service and process improvement.

Requirements

  • Bachelor’s degree in nursing or related field required
  • Prior experience in behavioral health or SUD
  • Experience with community-based case management
  • Behavioral health experience
  • Experience in behavioral, clinical, utilization management, home care, discharge planning, and/or case management required

Nice To Haves

  • Master’s degree in nursing, social work, or related field
  • Current NCOEMS certification as EMT, AEMT, or Paramedic

  • Identify barriers to care and collaborate with patients, families, and support networks to establish integrated plans of care
  • Participate in referral conferences, interagency meetings, treatment planning, and departmental initiatives
  • Ensure timely, accurate case management and documentation in accordance with established standards
  • Identify system and provider issues and contribute recommendations for service and process improvement
