About The Position
The Patient Navigator provides individualized service, navigating and supporting patients throughout the health system. He/she works in a dynamic health care environment with multiple departments to ensure timely screening, diagnosis, treatment and supportive services. S/he works in conjunction with the Nurse Navigator and other health care professionals to evaluate and address the needs of patients served to ensure timely access to services needed and optimize health and wellness.
Requirements
- Associates Degree preferred, Patient Navigator Certification preferred, or obtained within 1 year of employment
- Basic understanding of medical terminology, health care systems and health care financing required.
- Proficiency with a personal computer is necessary.
- Demonstrated fluency in English.
- Ability to navigate the health care system in effort to obtain necessary information.
