About The Position

Responsible for identifying critical resources for clients, helping them navigate through health care services and systems, and promoting client health.

Responsibilities

• Offers medical guidance and acts as a support for group.
• Guides patients and their families through the treatment process.
• Navigates patients through the diagnostic evaluation.
• Educates and supports each patient empowering them to make informed treatment decisions.
• Remains a support system throughout the patient's treatment.
• Performs other related duties as assigned.
