About The Position

Join a Mission-Driven Team at the Mass General Cancer Institute (MGBCI) as a Patient Navigator! As a Patient Navigator, you'll be at the forefront of patient advocacy, helping individuals access the care they need while working alongside a dedicated and collaborative team. Your contributions will directly support MGBCI’s mission to deliver world-class cancer care with empathy and excellence. Responsible for working closely with patients, families, and healthcare providers to ensure a seamless and patient-centered care experience by assisting with navigating the complexities of the healthcare system, coordinating resources, and providing support to improve patient outcomes.

Requirements

  • Bachelor's Degree required
  • Strong knowledge of healthcare resources, community services, and patient advocacy.
  • Excellent communication and interpersonal skills.
  • Ability to collaborate effectively with healthcare professionals across multiple disciplines and experiences.
  • Strong organizational and time management skills.
  • Familiarity with electronic health records and case management software.

Responsibilities

  • Lead and participate in the intake process for new patients, helping them navigate their journey into MGBCI with compassion and clarity.
  • Provide essential case management services—including financial assessments, scheduling, and medical records coordination—to support patients and their families.
  • Ensure timely and accurate documentation in the electronic medical record, meeting expectations set by our Medical Director and Administrative Manager.
  • Advocate for patients and their families, ensuring their needs and preferences are considered in the care planning process.
  • Serve as a liaison between patients, healthcare providers, and other relevant stakeholders.
  • Collaborate with the interdisciplinary healthcare team to coordinate patient care services, appointments, and follow-up plans.
  • Assist patients in understanding and adhering to their care plans.
  • Identify and connect patients with appropriate healthcare and community resources, such as support groups, financial assistance programs, and transportation services.
  • Educate patients and their families about their medical conditions, treatment options, and self-care strategies.
  • Ensure that patients are informed and empowered to actively participate in their healthcare decisions.
  • Maintain accurate and detailed documentation of patient interactions, care plans, and resource referrals.
