About The Position

Identify and connect with community members to assess their unique needs and guide them toward the most appropriate health and wellness services. Advocate for patients by assisting with medical appointments, accompanying them to visits, and simplifying the application process for vital healthcare programs. Provide compassionate, informal counseling and education to empower clients with the social support and knowledge they need to manage their health. Cultivate strong partnerships with community agencies and internal clinical teams to build a reliable network of resources for the people we serve. Document every patient interaction and outcome to ensure no one falls through the cracks and the impact of our services is clearly captured.

Requirements

  • High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
