About The Position

Reporting to the Operations Leader, the Patient Navigator is responsible for the coordination of services to participants in the program. In collaboration with health care providers, this position provides support, information, education and care coordination to participants and their families as well as liaising with hospital and community partners to ensure continuity of care and the achievement of optimal clinical outcomes. The Navigator participates in the development of standards and processes; monitors and evaluates services and acts as a clinical resource for physicians and other health care professionals, and the community in matters relating to their area of care. This position champions their program and works to establish and maintain links between the program, participants, health care providers, community agencies and care facilities. The Patient Navigator works within an interdisciplinary clinical framework in accordance with the BCCNM Standards of Practice and consistent with the mission, vision and values and established protocols of Providence Health Care.

Requirements

  • Registered Nurse with current practicing registration with the BC College of Nurses and Midwives or, in Residential Care and Mental Health a Registered Psychiatric Nurse with current practicing registration with the BC College of Nurses and Midwives.
  • Two years of recent related experience.
  • Ability to communicate effectively both verbally and in writing.
  • Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service.
  • Ability to work as a member in an interdisciplinary team
  • Ability to apply decision making and analytical skills
  • Ability to deal with others effectively.

Responsibilities

  • In collaboration with program staff, Consultants and Operations Leaders, participates in the development of program policies, procedures, standards and processes for the program and follow-up at Providence Health Care; evaluates and monitors service outcomes and recommends modifications to the program to improve services and the participant's experience.
  • Provides nursing care according to identified policies and procedures, technology and PHC standards.
  • Performs duties such as providing direction regarding the scheduling of diagnostic procedures and necessary assessment, interviewing and assessing patients and developing individualized care plans, based upon patients' specific care requirements, and reviewing all ordered test and consults, and reports abnormalities, specific assessment problems and/or concerns to appropriate medical personnel
  • Provides education to participants and their families by performing duties such as developing, implementing, evaluating and modifying education materials.
  • Assesses factors affecting patient/families learning and modifies education plans according to adult learning principles.
  • Ensures that patients receive all requisite education regarding their upcoming test/procedure and reviews patients' responses.
  • Acts as a resource to other nurses and healthcare professionals to support their role in teaching participants and works closely with members of other health care disciplines and physicians in coordinating patient education.
  • Identifies and provides patient and family with appropriate support, including initiating referrals to other members of the interdisciplinary team.
  • Performs related duties such as providing information and support to participants through their decision-making process regarding booking tests/procedures and follow-up procedure options, advocating on behalf of the participant and family to support their choices and needs, following up with participants, evaluating and researching outcomes and evaluating treatment processes, and ensuring a smooth and appropriate transition from the booking of tests/procedures to treatment.
  • Coordinates and completes data collection by performing duties such as developing data collection tools.
  • Provides input into departmental standards, protocols, policies and procedures as requested.
  • Participates in formulation, implementation and evaluation of Continuous Quality Improvement initiatives.
  • Performs other related duties as assigned.
