The Division of Gastroenterology at the Icahn School of Medicine at Mount Sinai is looking for a Patient Navigator. The Patient Navigator oversees a caseload of patients and acts as the primary liaison between the patient and the hospital system; this involves scheduling and helping to complete their colonoscopy. The Patient Navigator can also work as a lead Patient Navigator, which is a liaison between less experienced Patient Navigators and management. In this role, you are responsible for implementing the comprehensive care plan to each patient as determined upon enrollment in the program. The Patient Navigator ensures that the patient adheres to the established plan of care. As a Program, our work is focused on helping to reduce health disparities and ensure anyone and everyone can have access to the care they need. As an integral part of our colorectal cancer screening program, we are seeking a Patient Navigator to join our team to assist patients in completing their colorectal cancer screening and also serve as a mentor less experienced Patient Navigators.
