About The Position

Terry Reilly is seeking a Patient Navigator to join their team in Boise. This role works closely with outreach, medical providers, and primary care teams to provide short-term care coordination to improve patient health and well-being. The position aims to enhance quality and patient engagement by addressing barriers to accessing care and keeping patients active in their healthcare journey. The role requires critical thinking, analytical skills, and effective communication.

Requirements

  • High School Education or equivalent
  • 1 year experience in a primary care setting, social services, or community-based field.
  • CHW Certification or the ability to become certified within 1 year.
  • Good understanding of health care delivery including insurance eligibility, enrollments, attribution and use of a medical home.
  • Experience with case management for complex patients with barriers to care
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community both in person and over the phone.
  • Comfortable with and successful at public speaking.
  • Knowledge of community agencies and resources.
  • Skill in use of personal computers and related software applications, including but not limited to: Microsoft Office Suite, EHR and web applications.
  • Available for regular weekend and evening duties and multiple non-traditional work sites such as outreach events and home visits.
  • Ability to work independently with minimal supervision and use sound judgment.
  • Well-organized, creative, flexible, open-minded and able to manage several tasks simultaneously.

Nice To Haves

  • Bachelor's Degree in health promotion or other health related field, education, communication, or social services.
  • Experience working within a primary care setting.
  • Bilingual in English and Spanish.
  • Knowledge and experience with health promotion
  • Experience working with underserved and/or aging patient populations.
  • Experience with Medicare and major insurance carrier regulations, procedures, and benefit plans.
  • Experience using Electronic Health Record Systems (EPIC preferred), including scheduling.

Responsibilities

  • Provide short-term care coordination to improve patient health and general well-being.
  • Coordinate care and services available at Terry Reilly under the supervision of the outreach department.
  • Improve quality and increase patient engagement by helping patients address barriers to accessing care.
  • Keep patients active in their care.
  • Investigate and resolve discrepancies.
  • Connect patients with community agencies and resources.
  • Perform regular weekend and evening duties.
  • Work at multiple non-traditional work sites such as outreach events and home visits.
