About The Position

The Patient Navigator coordinates and supports all aspects of an assigned clinical service under appropriate supervision. This role serves as a vital liaison between patients, providers, departments, and community partners to ensure seamless care coordination and an exceptional patient experience. The Patient Navigator helps patients access the care and resources they need by identifying and addressing barriers throughout their healthcare journey. Through education, advocacy, and coordination, this role promotes patient engagement and supports adherence to treatment plans.

Requirements

  • High School Diploma, GED, or equivalent.
  • 3+ years of experience in patient navigation, healthcare scheduling, care coordination, or outpatient administrative support.
  • Experience using Epic Electronic Health Records (EHR).
  • Strong verbal and written communication skills.
  • Excellent organizational and prioritization abilities.
  • Self-motivated with the ability to work independently.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Exceptional customer service and patient advocacy skills.
  • Proficiency in Microsoft Office applications.
  • Commitment to compassionate, patient-centered care.

Responsibilities

  • Help patients navigate clinical services and overcome barriers to care.
  • Educate patients about diagnoses, treatment options, and available resources.
  • Inform eligible patients about clinical research opportunities and innovative healthcare technologies.
  • Provide compassionate guidance and support throughout the patient journey.
  • Serve as a liaison between patients, providers, departments, vendors, and community partners.
  • Coordinate and schedule departmental, provider, and informational meetings.
  • Follow patients throughout the continuum of care, including inpatient admissions.
  • Partner with care management teams to ensure smooth transitions of care.
  • Develop processes that address healthcare disparities and improve access to services.
  • Maintain relationships with referral sources and community agencies.
  • Collaborate with clinical teams to support patient engagement and adherence to care plans.
  • Reach out to patients who miss appointments and encourage re-engagement with care.
  • Schedule consultations, treatments, and support services within established guidelines.
  • Ensure timely access to care and coordinated scheduling experiences.
  • Coordinate patient education sessions, both in-person and virtual.
  • Provide educational resources and support to patients and families.
  • Document all patient communications and educational activities in the medical record.
  • Maintain accurate patient and clinical records.
  • Upload and manage documentation within Epic and other electronic systems.
  • Support service-specific data collection, reporting, and compliance activities.
  • Assist with support group coordination, educational programming, and meeting logistics.
  • Identify and coordinate guest speakers and educational resources.
  • Participate in outreach initiatives, community events, and awareness programs.
  • Provide administrative support to ensure efficient clinic operations.
