About The Position

The Patient Navigation Specialist will coordinate all aspects of the specific assigned clinical service. They will serve as a liaison with other departments and will interface with internal and external agencies, including diverse levels of medical and non-medical personnel, to ensure the optimal level of clinical coordination is achieved. This role involves assisting patients in navigating clinical services, overcoming barriers to access, and encouraging re-engagement with healthcare providers. The specialist will also maintain relationships with referral sources, collaborate with clinical staff for patient adherence, manage data and clinical information, and participate in outreach programs and community events. Additionally, they will assist in coordinating support groups and serve as a departmental liaison.

Requirements

  • High School Diploma/GED or equivalent OR 2 years of work experience.
  • Ability to navigate EPIC- Electronic Health Records software.
  • Ability to speak concisely and effectively communicate.
  • Self-Motivated.
  • Ability to prioritize work to meet deadlines.
  • Ability to function in a high-pressure environment and meet stringent deadlines for completing tasks.
  • Ability to handle multiple tasks simultaneously.
  • Strong customer service and patient relations skills.

Responsibilities

  • Assists patients in navigating clinical services provided to include assistance in overcoming barriers that may inhibit accessing these services.
  • Contacts No-show patients to encourage re-entry either to the clinical service or other clinical provider.
  • Maintains relationships with internal and external referral sources and agencies to facilitate patient referrals.
  • Collaborates with clinical staff and providers to ensure patients’ engagement & adherence to plan of care.
  • Maintains collection and storage of data & clinical information specific to service to include filing, upload of clinical documentation into EHR and electronic programs.
  • Participates, coordinates and schedule identified outreach programs and community events.
  • Assists in working with the coordination of support groups, to include attendance at meetings, identifying educational speakers and providing information concerning the support group to all patients and providers.
  • Serves as liaison and/or contact person for the department or office between and amongst other departments, patients, and vendors.
  • Coordinates and schedules departmental, informational and general staff and provider meetings.
  • Performs other duties as assigned and develops special projects as assigned by service program medical director and/or the Operations Manager.
