Navigators will serve as the primary point of contact for individuals referred to Tarzana Treatment Centers (TTC) through the Homeless Housing Recovery Program (HHRP) / CD3 system, including referrals from coordinated entry systems, county agencies, hospitals, community-based organizations, and other public and non-profit partners. Navigators are responsible for responding to referrals in a timely manner, conducting initial screenings and needs assessments, and actively coordinating access to appropriate housing and supportive services. This includes scheduling intakes, facilitating service linkage, and ensuring individuals are connected to TTC programs in alignment with HHRP/CD3 guidelines and timelines. Navigators will maintain accurate and up-to-date documentation in all required systems, track referral outcomes, and collaborate with internal teams and external partners to support housing stabilization and continuity of care. The role requires ongoing communication, follow-up, and advocacy to ensure participants successfully engage in services and progress toward housing goals.
