About The Position

A Patient Navigator is a person with direct knowledge of the local community who functions as a “guide” and offers assistance to community members in “navigating” the health care system and accessing related social and financial services to improve health outcomes. The Patient Navigator performs a variety of information dissemination, health education, clerical and medical support services within the clinic setting and in the community. A primary function of the navigator is to establish and maintain communication between patients, their families, care team members, and the health care and social service systems to help facilitate the care of individuals.

Requirements

  • High school diploma or GED.
  • Must maintain a current Cardio-Pulmonary Resuscitation certification, in compliance with corporate CPR Policy.
  • Effective verbal and written communication skills to communicate clearly and effectively with patients and medical providers.
  • Possess communication and other interpersonal skills that can ensure an ability to work with local people and organizations.
  • Ability to be trained to perform a variety of tasks.
  • Ability to complete work assignments with minimal supervision, thoroughness, timely, and efficiency.
  • Committed to ongoing learning, able to model the behavior and activities that will support both individual development and community capacity-building.
  • Must have a pleasant, patient-oriented attitude and a professional appearance.
  • Sensitivity and respect for the different cultures represented among all staff.
  • Demonstrated ability to set priorities for tasks to work effectively despite interruptions and under minimal supervision. Self-starter, reliable, and dependable.
  • Fluency in English (speak, read, and write).
  • Fluency in Spanish (speak, translate, read, and write).
  • Problem-solving skills and the ability to make routine recommendations.
  • Ability to maintain absolute confidentiality about health care and other patient/client information.
  • Must possess a current California driver's license, reliable means of transportation, appropriate insurance coverage, and a driving record acceptable to the NEVHC's insurance carrier (if required to drive on the job).

Nice To Haves

  • Previous experience in a health care setting is a plus.
