If so, Callen-Lorde wants to hear from you! We are seeking a Patient Navigator to join our dedicated team and play a vital role in supporting the needs of our patients living with HIV. The role places a strong emphasis on prevention and patient education, including HIV prevention strategies such as PrEP and PEP, long-acting injectable (LAI) treatments, and broader preventive care services. Experience or familiarity with sexual health education and adult patient care is highly valued.
Requirements
- High school diploma or GED required.
- CPR Certification Require
- Experience in social services, case management and/or entitlements work.
- Strong computer competency, including knowledge of Electronic Medical Records, Microsoft Office Programs, and internet research savvy.
- Strong customer service orientation with an understanding of patient-led treatment.
Nice To Haves
- Experience working with individuals with a spectrum of chronic illness conditions preferred
- Knowledge of Microsoft office programs and utilization of these tools for scheduling and communication.
- Strong communication and interpersonal skills.

Responsibilities
- Act as a patient advocate, ensuring patients' biopsychosocial needs are identified, addressed, and connected to appropriate services.
- Provide field-based case management and adherence support services to HIV-positive patients.
- Work collaboratively with care coordinators, medical providers, and behavioral health professionals to facilitate patient retention and adherence to primary medical care.
- Assist patients in navigating the healthcare system, overcoming barriers to care, and connecting them with essential resources.
- Advocate for patients' rights and ensure they receive culturally competent care.
- Document patient interactions and progress notes to ensure continuity of care.
- Contribute to a team-oriented environment, fostering a supportive and inclusive space for patients.
