About The Position

The Patient Health Coordinator meets with patients living with HIV to conduct an intake, link them to services and provide adherence education. This position will assist in transportation arrangements for patients to services as needed. Coordinates program and patient services. Help to resolve patient problems or issues. Facilitates educational/life skills groups.

Requirements

  • Bachelor’s Degree in health-related field AND 2 years’ experience in an administrative role.
  • OR Licensed Practical Nurse (LPN) AND 3 years of experience serving individuals impacted by HIV/AIDS AND 2 years’ experience in an administrative role.
  • OR Registered/Certified Medical Assistant (MA) with 8 years of experience serving individuals impacted by HIV/AIDS AND 5 years’ experience in an administrative role.
  • Excellent communication, organizational and writing skills.
  • Proficient with Microsoft office. Ability to work in multiple system – tech-savvy
  • Prior health/medical and /or social services experience required.
  • Strong/demonstrated case management and group facilitation skills
  • An automobile, valid Driver’s License and vehicle insurance coverage is required.

Responsibilities

  • Complete client paperwork including intake, nutritional screen, initial adherence, and ADAP forms, as needed.
  • Complete assigned reports as requested (i.e., OCR, CDPH, transportation, etc)
  • Maintain client level reporting system and perform data entry, as needed.
  • Assist in the coordination and oversight of Transportation services.
  • Provide supervision, training, assistance and support to assigned staff.
  • Complete required external and internal program reports within timely manner.
  • Participate in quality management program and activities.
  • Maintain appropriate documentation and ensure patient charts are in compliance.
  • Assist in quality assurance activities and conducting chart audits for HIV program
  • Network with HIV community to market and enhance program services.
  • Participate in HIV workgroups, networks, councils or committees as identified.
  • Coordinate patient services, i.e., referrals for dental, mental health, etc.
  • Facilitate individual and group educational sessions (including securing speakers, space, food and transportation for participants).
  • Monitor and oversee the access of care for newly diagnosed HIV+ and return to care patients into CCHC primary care services
  • Provide adherence education for patients that are newly prescribed or have a change in Highly Active Anti-retroviral Therapy (HAART).
  • Collaborate with CCHC’s Counseling and Testing Initiative and CCHC’s DIS on Partner Services for new clinic positives.
  • Prepare current and updated HIV educational and informational resources for distribution to patients.
  • Inform and support patients in empowerment and advocacy opportunities.
  • Conduct, develop, monitor or support during annual patient focus groups.
  • Assists in resolving patient concerns/issues.
  • Provide support and assistance to CCHC’s CAB, including conducting training, setting up speakers, assisting with coordinating community health events and providing technical assistance.
  • Conduct outreach to patients to link them to care.
  • Maintain ongoing working relationships and collaboration with CCHC’s HIV team members and programs.
  • Attend all program related meetings and trainings.
  • Provide assistance to supervisor as needed.
  • Travel to other clinic locations to meet with patients as needed.
  • Other duties as needed/assigned within scope of program.
