About The Position

The Patient Flow, Admissions & Discharge RN plays an essential role in ensuring safe, efficient, and seamless patient transitions across the continuum of care. This innovative position focuses on coordinating and completing patient admissions and discharges in all hospital areas, helping patients and families navigate their hospital experience with confidence and clarity. By leading key admission and discharge activities, this role allows bedside nurses to spend more time delivering direct patient care while improving patient flow, enhancing throughput, and supporting positive patient outcomes.

Requirements

  • Graduate of an accredited or state-approved Registered Nursing program.
  • Current Massachusetts Registered Nurse (RN) license.
  • Two years of clinical nursing experience.
  • Strong clinical assessment, communication, and organizational skills.
  • Demonstrated commitment to providing safe, compassionate, high-quality patient care.
  • Current Massachusetts Registered Nurse (RN) license required.

Nice To Haves

  • Bachelor of Science in Nursing (BSN) strongly preferred.
  • Experience in one or more of the following areas: Medical-Surgical Nursing, Care Coordination, Transitions of Care, Patient Flow or Throughput Operations
  • Ability to effectively balance exceptional patient care with operational efficiency and patient movement initiatives.

Responsibilities

  • Complete comprehensive patient admission assessments and documentation.
  • Perform medication reconciliation and identify potential medication-related concerns.
  • Coordinate and facilitate timely patient admissions and discharges.
  • Provide individualized patient and family education regarding hospitalization, care plans, and discharge instructions.
  • Collaborate closely with physicians, case managers, nursing staff, and ancillary departments to support effective care transitions.
  • Advance patient throughput initiatives and care coordination efforts.
  • Help reduce administrative demands on bedside nursing teams, creating greater capacity for direct patient care.
  • Promote a culture of safety, efficiency, and patient-centered excellence throughout the hospitalization journey.
