About The Position

CareBridge Health, a member of the Elevance Health family, is seeking a Patient Enrollment Specialist – Transitions of Care. This role is responsible for managing new and existing patients, explaining services, enrolling them into appropriate programs, and providing support by addressing inquiries and scheduling appointments. The specialist will enter documentation into designated systems and must be proficient in handling multiple communication channels (calls, texts, facsimiles, electronic queues) simultaneously while taking notes and speaking with customers. This position requires the ability to multitask in a fast-paced, high-pressure environment, strong communication skills, attention to detail, critical thinking, problem-solving abilities, and empathy. The role is primarily virtual, offering flexibility, but requires occasional in-person training and may require working from the office as needed. Associates will have a structured work schedule with potential for occasional overtime.

Requirements

  • Requires a HS diploma or equivalent and 1 year of experience in a call center, sales, health care or customer-focused environment; or any combination of education and experience which would provide an equivalent background.
  • Ability to multi-task, including handling calls, texts, facsimiles, and electronic queues, while simultaneously taking notes and speaking to customers.
  • Proficient in maintaining focus during extended periods of sitting and handling multiple tasks in a fast-paced, high-pressure environment.
  • Strong verbal and written communication skills, both with virtual and in-person interactions.
  • Attentive to details, critical thinker, and a problem-solver.
  • Demonstrates empathy and persistence to resolve caller issues completely.
  • Comfort and proficiency with digital tools and platforms to enhance productivity and minimize manual efforts.

Nice To Haves

  • Bilingual in English and any of the following languages preferred: Spanish, Russian, Chinese, Vietnamese, Arabic, and Korean.

Responsibilities

  • Reviewing and prioritizing patient census.
  • Reaching out to patients to explain services and enrolling them into the appropriate programs based on the line of business.
  • Providing support by addressing inquiries and scheduling the initial and annual appointments.
  • Entering documentation into the designated systems.
  • Handling calls, texts, facsimiles, and electronic queues simultaneously while taking notes and speaking to customers.
  • Performing other duties as assigned.
