As a Patient Educator, you’ll be the bridge between information and action, working closely with our care team to provide personalized health education and support. From conducting point-of-care testing to assisting patients with Legacy’s services, you’ll play a pivotal role in shaping healthier communities.

Requirements

  • High School Diploma or equivalent
  • 3 - 5 years of relevant experience
  • Experience providing direct education to clients of diverse cultures and backgrounds in one-on-one and/or group settings.
  • Experience in a healthcare setting.
  • Experience with computers and computer programs.
  • Ability to provide point-of-care testing within 3 months if/as required per site assignment.
  • Ability to become phlebotomy trained within 3 months of hire if/as required per site assignment.
  • Ability to demonstrate core competencies within 3 months of hire.
  • Bilingual English/Spanish.
  • Experience working in an electronic health record system.
  • Experience working with people living with or at-risk for HIV.
  • Experience working with sexual minorities and gender non-binary persons.
  • Experience providing education about pregnancy and/or the post-partum period.
  • Experience working with people who have chronic health conditions.

Nice To Haves

  • Bachelor’s Degree in a related field with a minimum of 1 year of relevant experience preferred.

Responsibilities

  • Implement tailored health education programs in one-on-one, co-visit, and group settings to empower patients in managing health conditions and promoting wellness.
  • Help patients set achievable health-related goals based on their behavioral, social, and medical needs.
  • Conduct point-of-care testing, including blood glucose, HIV testing, pregnancy testing, and risk assessments.
  • Support patients in navigating Legacy’s healthcare services to meet their health goals.
  • Document patient services in electronic health records with accuracy and attention to detail.
  • Collaborate with an interdisciplinary clinic/care team to foster a positive, team-focused environment.
  • Develop engaging presentations, handouts, and tools to effectively communicate with patients and community members.
  • Participate in designing and evaluating new health education curricula driven by patient and community needs.
  • Meet performance metrics, including grant and contract requirements.
  • Attend clinic, team, departmental, and agency meetings, as well as trainings and workshops, which may require travel to various Legacy clinics.
  • Take on additional tasks as assigned to support the clinic’s mission and goals.
