About The Position

Under the supervision of the LPN, RN, or nurse leader, the Patient Care Tech (PCT) performs patient-centered, evidence-based nursing care activities to meet the physical and psychosocial needs of the patient. Through collaborative practice with all members of the care team, is responsible for patient outcomes that meet the high quality of care provided by ThedaCare. Contributes to the goals of the department by being accountable for the delivery of compassionate, safe care and demonstrated commitment to continuous growth and professional development. We are seeking a compassionate and skilled Registered Nurse/Patient Care Technician to join our specialized, 10 bed, inpatient rehabilitation unit. This is located within the ThedaCare Orthopedic Spine and Pain Hospital. We provide specialized care to patients recovering from stroke, orthopedic procedures, neurological conditions and other life altering trauma. You would be joining a collaborative unit that includes a team of Physical Therapists, Occupational Therapists, Speech Therapists and Physiatrists. This role focuses on providing high quality, patient-centered care. We are dedicated to helping patients regain independence and improve quality of life.

Requirements

  • Must be 18 years of age required expect for Peabody Manor, The Heritage or Juliette Manor
  • American Heart Association HealthCare Provider Life Support (BLS)

Nice To Haves

  • High School diploma or GED preferred
  • Previous healthcare experience preferred

Responsibilities

  • Performs delegated patient care activities using evidence-based practice and specific competencies for the patient population served.
  • Checks patients’ vital signs, height, and weight, as needed, based on patient type.
  • Provides compassionate care and listens to patients’ concerns and guides them by answering any questions or concerns.
  • Identifies and escalates changes in patient status or patient risks to prevent crisis.
  • Collects clinical data and information on patients and reports to care team.
  • Repositions, transfers, ambulates and transports patients.
  • Assists patients with basic needs (such as hygiene, eating, using the restroom, and dressing).
  • Obtains and reports patient intake and output.
  • Performs electrocardiograms (ECGs) as directed.
  • Performs phlebotomy and specimen collection.
  • Provides care that is age appropriate, culturally specific, and developmentally appropriate in an environment that promotes patient safety.
  • Demonstrates professional and therapeutic relationships with patients, families, team members, and providers.
