About The Position

DaVita Kidney Care is expanding its services by bringing industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services. This initiative aims to reduce the burden on patients who are transported for dialysis three times a week and to decrease avoidable healthcare costs. Teammates in this role will leverage DaVita's resources to develop the infrastructure and grow this new service line. The ideal candidate thrives in a start-up environment, is passionate about improving patient lives, and has a proven ability to get things done. Comfort with autonomous work and collaboration with cross-functional teams is essential. In this role, you will be responsible for the complete ongoing care of each patient to ensure their dialysis treatment stays on track, working in conjunction with dialysis RNs. The primary work schedule is Full-Time, based at the site dialysis den within Northridge Health and Rehabilitation, with potential support for additional surrounding sites as needed. Operations run Monday through Saturday, with scheduled days varying based on market needs. This is a complex but fulfilling service line where meeting and exceeding clinical outcomes and multitasking are expectations to meet the needs of the service, patients, and SNF partners. You will provide comprehensive patient care in an independent setting, constantly using your critical thinking skills to monitor patients before, during, and after dialysis treatment, including measuring and recording stats, patient observations, and hemodialysis machine setup. The role offers the satisfaction of witnessing the positive impact of your care on patients' lives.

Requirements

  • High school diploma or equivalent
  • CCHT Certification
  • Physical and mental ability to work long hours (some shifts are 12+ hours) on your feet
  • Basic computer skills and proficiency in MS Word and Outlook

Responsibilities
