Patient Care Technician DSD - PRN
About The Position
DaVita Kidney Care is bringing its industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services. This role is crucial for providing comprehensive patient care in an independent setting, monitoring patients before, during, and after dialysis treatment. The position involves multitasking to meet the needs of the service, patients, and SNF partners, requiring autonomy and critical thinking. Teammates will have access to DaVita's capabilities and resources to develop and grow this new service line. The PCT will be based at our site dialysis den within Northridge Health and Rehabilitation and may be asked to support additional surrounding sites depending on business needs. Operations function Monday through Saturday.
Requirements
- High school diploma or equivalent
- CCHT Certification
- Physical and mental ability to work long hours (some shifts are 12+ hours) on your feet
- Basic computer skills and proficiency in MS Word and Outlook

Responsibilities
- Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
- Work in conjunction with dialysis RNs to provide dialysis care.
- Provide comprehensive patient care in an independent setting.
- Monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup.
- Commit to improving patient health through clinical goal setting and quality improvement initiatives.
