About The Position

Located in the Phoenix, Ariz., suburb of Chandler, the Chandler Regional Medical Center is a 429-bed, not-for-profit hospital with more than 2,600 team members and almost 1,000 doctors on staff representing all major specialties. Chandler Regional joined the Dignity Health system in 1999, but has been part of the community for more than 50 years. The hospital includes a Level I Trauma Center, a Heart and Vascular Center, a Wound Healing Center, a Family Birth Center and many additional services. Dignity Health Chandler Regional, Mercy Gilbert, and Arizona General Hospital Mesa awarded $422,000 in grants to support five community projects led by 15 local organizations. Since 1991, Dignity Health's Community Health Improvement Grants program has provided $91 million to nearly 3,800 health projects, focusing on vulnerable and underserved populations.

Requirements

  • High School Graduate General Studies upon hire and 1 year acute care experience Required or High School GED General Studies upon hire and 1 year acute care experience Required
  • Completion of nursing assistant program or completion of first block of nursing program
  • Certified Nursing Assistant: AZ, upon hire
  • Basic Life Support - CPR, upon hire

Responsibilities

  • Performs patient-related care tasks (direct care, bathing, oral care, feeding).
  • Assists patients with activities of daily living and provides direct patient care under supervision of Nurse.
  • Obtains and records patient vital signs.
  • Monitors and records daily intake and output.
  • Obtains specimens, including non-invasive body fluids and blood specimens.
  • Monitors and performs actions to decrease patient length of stay, including but not limited to: use of the computerized Bed Tracking System, performing timely designated discharge duties, and communicating discharge information to appropriate staff.
  • Assists Physician/Nurse with diagnostic tests, clinical procedures, and other tasks as requested.
  • Communicates patient needs and observed/reported changes in patient condition to supervising Nurse or Physician.
  • Documents all interactions in patient's medical record.
  • The PCT may assist nursing in the retrieval and delivering of medications per the unit standard.
  • Assists the nurse/physician during examination, treatment, and procedures as requested.
  • Documents clinically relevant findings in the patient's record according to hospital protocol.
  • Completes hospital forms per protocol
  • Assists with patients' discharge/admission/transfer.
  • Orients patient to room/routine
  • Obtains vital signs/specimens as directed.
  • Demonstrates respect for protection of patients' rights.
  • Collects laboratory specimens according to hospital protocol
  • Reports significant observed or alterations in the patients' condition to the RN
  • Collaborates with the RN in implementation of the plan of care
  • Addresses physiological, psychosocial, and spiritual needs of the patient and provides for patient safety and comfort at all times
  • Clearly documents all interventions and records reading in an accurate and timely manner.
