About The Position

Reporting to the Lead, Patient Support Programs (PSP) & Preferred Pharmacy Network (PPN), the Bilingual PSP Case Manager provides national support to Canadian patients living with chronic or rare conditions by ensuring patients have access to their prescribed treatments in a timely manner and improve their quality of life. The Bilingual PSP Case Manager provides enrolment and reimbursement assistance, manages financial assistance activities, provides patients with disease state education, performs case management and healthcare services to patients, their dependents and the patient’s direct circle of care for specific prescribed molecules. The Case Manager manages Special/Prior Authorizations at both the private and public payer-levels with submissions pertaining to obtaining drug reimbursement and coverage through private and provincial drug formularies.

Requirements

  • Fully Bilingual in French and English (fluently read, write and speak) - Strongly preferred.
  • Possess strong clinical expertise and experience in a range of health conditions
  • A university degree in a health-related field
  • 5+ years PSP or case management experience ideally
  • Understanding of provincial and private coverage criteria and how to navigate the Canadian drug coverage landscape
  • Familiarity with the Special / Prior Authorization process with both public and private insurance
  • Familiarity and knowledge of Health Canada Pharmacovigilance requirements
  • Proven ability to work effectively as part of an interdisciplinary team and demonstrate competence in performance of case management, providing patient education, virtual assessments / interventions and documentation
  • Excellent organizational, problem-solving, decision making, conflict resolution and critical thinking abilities
  • Excellent interpersonal, listening and communication skills (written and verbal), ability to direct conversations and target specific issues
  • Tech savvy with fundamental knowledge of Microsoft Office and other tools
  • Self-regulation of time management and the ability to multi-task and adhere to deadlines
  • Familiarity with Provincial (e.g. Personal Health Information Protection Act) and Federal (e.g. Consumer Privacy Protection Act) privacy laws
  • Proficiency in English is required for this position. As part of this role, you will be required to communicate with colleagues or customers who use English as their primary language. Responsibilities

  • Assist prescribing physicians with the completion of patient enrolments, gathering all pertinent information to complete private and/or provincial insurance forms
  • Liaise with insurance and healthcare providers as well as provincial governments as necessary to determine coverage options
  • Review patient charts (paper or electronic)
  • Complete welcome call activities with the patient according to defined program Standards of Practice (SOP)
  • Facilitate any required communication with physicians, patient support programs, or other healthcare professionals in the patient’s circle of care
  • Work closely with the manufacturer’s sales resources to support specific physician and staff needs
  • Offer and provide educational training to patients and physicians on specific product’s administration as aligned to the approved Health Canada product monograph
  • Act as a central resource for program stakeholders, which includes but not limited to physicians, nurses, patients, clinics, pharmacists, lab technicians, etc.
  • Ensure patient records are meticulously maintained and data is accurate in order to meet program reporting requirements
  • Leverage Pharmacovigilance knowledge and identify Adverse Events (AEs) / Product Quality Complaints (PQCs) and report according to predefined Health Canada and manufacturer expectations
  • Utilize nursing processes, through critical thinking, problem solving and decision making to proactively manage assigned patients who require case management services
  • Effectively navigate, teach, counsel, coach, support, direct and advocate as required on specific diseases, therapies and disease management
  • Provide adherence and persistency case management as required by the program
  • Utilize multiple software applications including CRM
  • Participate in quality activities and support continuous improvement of programs
  • Ensure compliance with provincial & federal privacy legislation
  • Participate in on going internal and/or external continuing education activities
  • Adhere to GreenShield Pharmacy’s Polices, Processes and Procedures
