About The Position

Nebraska Methodist Health System is seeking a part-time Patient Care Pharmacist for their Meds to Bed/Retail department at Methodist Hospital. This role involves providing medication therapy management services in collaboration with other healthcare providers to ensure safe, appropriate, cost-effective, and efficient drug therapy. The pharmacist will maintain competency in drug therapy for all age groups and patient dispositions, including inpatient, ambulatory, and outpatient. Responsibilities include interdisciplinary patient care rounds, medication reconciliation, monitoring, order management, dispensing, patient education, drug information, and drug dosing. A primary focus will be dispensing outpatient medications to discharging patients and providing patient education. The role also involves providing clinical and distributive tasks for safe medication use, and training students and residents. Proficiency with technology and workflows associated with medication management, such as automated dispensing cabinets, retail software, USP <797> IV requirements, and clinical decision support software, is essential. The pharmacist must ensure compliance with all relevant policies, rules, regulations, and state and federal laws.

Requirements

Bachelor’s degree in Pharmacy or doctor of pharmacy (PharmD) degree required.
Must be at least 18 years of age with no current drug or alcohol addition/convictions of any drug related misdemeanor or felony.
Current active Pharmacy Licensure (PH) for work state required.
Maintain competency in pharmaceutical knowledge and skills.
Effective verbal and written communication skills.
Ability to prioritize time and tasks according to patient needs and departmental duties.
Ability to supervise certified pharmacy technicians and pharmacist interns in departmental tasks and duties.
Ability to practice pharmacy with integrity towards coworkers and patients.

Nice To Haves

Two years of health care pharmacy experience preferred.
American Heart Association or American Red Cross Basic Life Support (BLS) preferred.
American Heart Association Advanced Cardiac Life Support (ACLS) preferred.
American Heart Association Pediatric Advanced Life Support (PALS) preferred.
Completion of an ASHP accredited residency training program and/or certification through a certifying program (Board of Pharmacy Specialties, etc.) preferred.

Responsibilities

Review medication orders for appropriate dose, duration, frequency, dosage form, indication, allergy, interactions, and contraindications.
Ensure medication regimen is consistent with organizational medication use guidelines, policies, or other evidence-based guidelines.
Adjust medication regimen based on approved protocols and/or consultation with the interdisciplinary team.
Monitor medication therapy for potential or actual drug interactions and outcomes.
Work as an active member of the multidisciplinary team and collaborate with healthcare providers within decentralized patient care areas.
Participate in interdisciplinary patient care rounds and oversee pharmacy resident and student participation.
Perform and/or review medication histories, comparing prescriber’s orders to the patients active medication list(s), and work with prescribers to make changes as necessary.
Document activities related to medication reconciliation in the medical record across patient’s transition of care.
Complete discharge medication review, facilitate the discharge process, including discharge prescriptions, and/or provide discharge education.
Ensure compliance with the health system inpatient drug formulary.
Complete medication profile review when staffing a clinical area, reviewing factors such as age, sex, health, weight, renal & hepatic function, pregnancy and lactation status, therapeutic duplication, available laboratory values, and appropriateness of antimicrobial therapy.
Coordinate and participate in the preparation and distribution of medications, including communication of pending issues/transactions with team members and other health care professionals.
Coordinate drug delivery and documentation as appropriate.
Complete and pass competencies associated with sterile, non-sterile, and hazardous drug compounding management.
Understand safety precautions, including hazardous materials handling and proper precautions against blood borne pathogens.
Monitor that the medication use system is providing the right patient, the right drug, the right dose, the right route, and the right time with the right outcomes.
Serve as a liaison for inpatient patient information for outlying patient needs (retail pharmacies, home health, physician clinics, etc.).
Identify and report adverse drug events (adverse drug reactions and medication errors) in the organizational reporting system.
Assure compliance with health system, hospital, and departmental policies and procedures.
Monitor compliance with state and federal laws and regulations related to the practice of pharmacy.
Participate in departmental and hospital committees as assigned.
Provide direct supervision of certified pharmacy technicians, pharmacist interns, and pharmacy students and assist with their education, training, and development.
Provide drug information/education to health care providers and patients upon request.
Provide in-services and lectures to health care team members as assigned.
Precept and evaluate pharmacy residents and PharmD students on residency/clerkship rotations.
Provide drug information and disease prevention programs to consumer and patient groups as needed.
