About The Position

As a Patient Care Manager – Ambulatory Services (PCM‑AMB) at our Newport News, VA location, you are a key leader within the multidisciplinary treatment team. This role is responsible for overseeing high‑quality, patient‑centered care in a transitional living or outpatient setting. The Patient Care Manager provides direct supervision, guidance, and mentorship to Patient Care Specialists (PCS I–III) and/or Patient Recovery Coordinators (PRC), ensuring consistency, safety, and excellence in frontline care while maintaining regulatory compliance and operational standards. Requirements

  • Associate degree from an accredited college or university in a behavioral science discipline plus one (1) year of experience in a behavioral health setting, or High school diploma or equivalent plus three (3) years of experience in a behavioral health setting
  • Minimum of one (1) year of experience supervising and leading a team
  • Strong understanding of addiction treatment and the recovery process, including medication‑assisted treatment
  • Current CPR/First Aid certification (may be obtained upon hire)
  • Current MANDT or CPI certification (may be obtained upon hire)
  • Valid driver’s license in good standing
  • Ability to travel up to 50% as needed

Nice To Haves

  • Undergraduate degree from an accredited college or university
  • Four (4) or more years of experience working in behavioral health
  • Two (2) years of experience supervising and leading a team

Responsibilities

  • Provide direct supervision, mentorship, and leadership to PCS and PRC teammates
  • Support recruitment, onboarding, retention, and performance development of patient care teammates
  • Promote accountability, engagement, and a positive team culture
  • Ensure role clarity and skill development through regular supervision and individual development planning
  • Maintain appropriate staffing and scheduling in accordance with program needs and Pinnacle standards
  • Manage accurate timekeeping and bi‑weekly payroll submission
  • Conduct 90‑day and annual performance reviews with measurable goals
  • Investigate teammate concerns and collaborate with HR and leadership on resolution
  • Plan, lead, and document team meetings and trainings to support quality improvement and regulatory readiness
  • Assist with admissions, transfers, and discharges, including patient monitoring, searches, observed drug screens, and transportation as needed
  • Collaborate with clinical and medical teams to support trauma‑informed care and crisis prevention
  • Oversee patient belongings searches, heat treatment, documentation, and secure storage of non‑permitted items
  • Manage patient cash balances and ensure accurate documentation and audits
  • Oversee patient self‑administration of medication procedures and vital sign collection in collaboration with nursing staff
  • Support PCS/PRC‑led recovery groups, meetings, and recreational activities
  • Oversee grocery budgets, meal planning, and ordering processes
  • Manage ordering and organization of patient care and program supplies
  • Assist patients without resources in meeting basic needs using available supports
  • Monitor documentation for accuracy, timeliness, and compliance, including EHR entries and incident reports
  • Ensure safety protocols, infection control measures, and facility cleanliness are maintained
  • Monitor census accuracy and communication tools related to patient occupancy
  • Coordinate and approve transportation arrangements when staff are unavailable
  • Uphold professional boundaries and ethical standards across the department
  • Serve as a liaison between patient care teams and other departments
  • Participate in daily huddles, treatment team meetings, and leadership meetings
  • Serve as Administrator on Call as scheduled
  • Train teammates on the PCS Playbook and Pinnacle Treatment Models
  • Perform all PCS/PRC duties as needed to support operations
  • Perform other duties as assigned
