Patient Care Liaison
About The Position
The Patient Care Liaison will manage relationships with insurance payers and MLTC (Managed Long-Term Care plans). This role involves verifying patient insurance eligibility, benefits, and coverage, as well as securing and tracking service authorizations for home care services. The Liaison will also resolve billing, claims, and reimbursement issues, stay informed of payer policies and regulatory requirements, and collaborate with internal teams to ensure uninterrupted patient care. Additionally, they will monitor authorization and reimbursement activity for process improvements and support community outreach and health plan marketing activities.
Requirements
- High School or GED required
- Bi-Lingual English/Spanish Required

- Previous home health care or home care experience highly desirable
