About The Position
Avita Health System is seeking a dedicated Patient Care Coordinator (PCC) to join their Nursing Administration Department at the Ontario location. The PCC will be responsible for assessing, planning, implementing, and evaluating the nursing care of patients within nursing units. This role ensures continuity of care between shifts by providing direct care and supervising nursing assistants, LPNs, and supportive staff. The PCC also participates in patient and family teaching and acts as a patient advocate, aligning with the organization's mission.
Requirements
- Current licensure as a Registered Nurse in the State of Ohio.
- Current BLS, ACLS, PALS certifications.
- Successful completion of Neonatal Resuscitation certification.
- Two (2) or more years of previous nursing experience.
Nice To Haves
- Previous management/supervisory experience.
- Bachelor’s degree in nursing (BSN).
Responsibilities
- Assesses, plans, implements, and evaluates the nursing care of patients within the nursing units.

- Supervises the care given by nursing assistants, LPNs, and supportive staff members.
