About The Position

The Patient Care Coordinator is responsible for scheduling services; including scheduling/rescheduling patients, new and returning patients. Verifies insurance information and registers the patient. Other office duties include; performing updates in the Epic, downloading glucometers, assisting with patient weights, coordinating virtual classes and support groups, updating patient ques. Requirements

  • AA Degree preferred
  • Two Years medical office experience
  • Excellent communication skills.
  • Excellent phone and computer skills.
  • Working knowledge of medical terminology.
  • Working knowledge of insurance terminology.
  • Strong organization skills and attention to detail.

Responsibilities

  • Greets patients and manages phone calls
  • Provides information on various department clinical programs to patients during patient scheduling/intake call
  • Schedules new and returning patient appointments
  • Coordinates virtual classes and support groups
  • Downloads and prints patient glucometer and continuous glucose monitor data
  • Assists with getting patient weights
  • Creates patient education packets for various clinical programs both inpatient and outpatient
  • Manages referral que and generates referrals when appropriate
  • Sends communication to referring physician offices when appropriate
  • Scans patient related documents for various clinical programs
  • Requests relevant laboratory data in preparation for patient visits
  • Assists the bariatric patient care advocate with guiding patients through the bariatric program
  • Builds customer relations through positive interaction with patients and other hospital departments
  • Consistently and accurately registers patients in Epic with all required information and keeps up with new registration requirements
  • Responsible for obtaining insurance and/or employer authorization as applicable.
  • Responsible for explaining benefits and co-payments to patients as applicable.
  • Responsible for subpoena and medical record requests, locating medical reports in various systems.
  • Ensures the daily review of email system for department communications.
  • Participates in department in-service programs.
  • Performs education modules monthly and HealthStream education when due.
  • Assists with inventory and replacement of supplies by ordering or notifying supervisor.
  • Attends huddles and mandatory in-service meetings.
  • Accepts assignment of other duties as needed, receives training and/or orientation.
