About The Position

Centralus Health, an affiliation of Arnot Health and Cayuga Health, is seeking an energetic and motivated individual for a newly created position. The Care Coordinator will play an integral role in promoting safe patient care transitions. This role involves collaboration with providers and practices (inpatient and outpatient), hospitals, home care agencies, and community-based organizations to ensure collaborative and consistent patient care delivery. The ideal candidate will possess strong analytical, problem-solving, critical thinking, and organizational skills, with the ability to function independently and with flexibility and integrity. The Care Coordinator also supports departmental efficiencies for team members and supervisors, requiring the ability to prioritize, coordinate, and manage multiple projects with high quality and productivity.

Requirements

  • Associate's Degree preferred, or High School Diploma with 5+ years of relevant experience in healthcare.
  • Experience in the acute care setting, or related healthcare field, is preferred.
  • Demonstrated knowledge of Microsoft products and zoom.
  • Ability to stand, sit or ambulate for long periods.
  • Ability to mobilize freely around units, transport self to site locations, and perform required data entry into EMR and alternate data management systems.
  • Ability to transport self to customer base.
  • Strong analytical, problem solving, critical thinking, and organizational skills.
  • Ability to function independently.
  • Flexibility and personal integrity.
  • Ability to prioritize, coordinate, and simultaneously maintain multiple projects with high level of quality and productivity.

Responsibilities

  • Assess the patient's health history and facilitate insurance navigations and primary care engagement.
  • Demonstrate understanding of palliative/hospice/home care referral requirements and facilitate referrals.
  • Demonstrate knowledge of and collaborate regularly with community-based organizations and partners.
  • Appropriately document client interactions and intended plan of care.
  • Routinely communicate with patient family/caregivers/decision makers.
  • Monitor and escalate issues affecting patient safety and/or length of stay.
  • Intervene to facilitate patient progression and safe care transitions.
  • Collaborate with ancillary personnel (e.g., physical therapy, pharmacy, diabetes education) to support patient care needs.
  • Attend multidisciplinary rounds and work with the team to develop safe/sustainable transitions of care.
  • Facilitate patient access to appropriate providers and service organizations.
  • Support administrative functions of the Clinical Resource Management Team.
  • Recognize and respond to opportunities for improvement.
