About The Position

InterCommunity is a Federally Qualified Health Center Look-Alike (FQHC LA) committed to providing accessible, compassionate care to everyone — regardless of life situation or ability to pay. We offer same-day primary care and a wide range of behavioral health services across our community health centers. Our Addiction Services Division provides a full continuum of care, including primary care integration, residential detox and treatment, outpatient mental health and substance use services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services, and social rehabilitation. We are seeking a dedicated and knowledgeable Care Coordinator to join our primary care team. The ideal candidate will have a medical background and a thorough understanding of healthcare practices. This role will primarily focus on coordinating referrals between primary and specialty care, including imaging, procedures, and transportation services, ensuring our patients receive seamless, comprehensive care.

Requirements

  • Knowledge of medical terminology, procedures, and medications
  • Proficiency in using electronic medical records and other healthcare software.
  • Commitment to continuous learning and professional development.
  • High school diploma or equivalent.
  • At least 1-2 years of experience working in a medical setting.

Nice To Haves

  • Completing a medical assistant program (CCMA, RMA) is preferred.

Responsibilities

  • Schedule patient appointments and coordinate referrals to specialists including imaging services, ensuring timely follow-up and completion of all referrals.
  • Arrange transportation for patients who require assistance getting to their appointments.
  • Facilitate communication and care by providing correspondence to patients, specialists, and other healthcare providers as needed.
  • Obtain prior authorizations for imaging, medications, and other services, ensuring that all documentation is completed accurately.
  • Document referrals, tracking patient progress and outcomes, and ensuring that all patients receive the necessary support.
  • Communicate regularly with patients to explain their care plans and address any questions or concerns, promoting patient engagement and understanding.
  • Participate in agency-sponsored in-services, workshops, conferences, and meetings to enhance professional skills and stay informed about current medical practices and trends.
  • Perform essential medical assistant duties, including rooming patients, taking vital signs, updating electronic medical records, conducting phlebotomy, and performing point-of-care testing as needed.
  • Assist at the front desk as required, which includes answering phones, checking patients in and out, and scanning/indexing documents.
  • All agency staff are required to attend all mandatory department/agency meetings and trainings.
