About The Position

UW Medical Center Montlake - Heart Institute has an outstanding opportunity for a Patient Care Coordinator. This role is crucial in providing a full range of heart healthcare and prevention services, including general cardiology, cardiac diagnostic imaging, cardiac surgery, arrhythmia management, interventional cardiology, advanced heart failure management, heart transplant, and other advanced services. Requirements

  • High school graduation or equivalent AND two years of experience in the program specialty OR equivalent education/experience

Responsibilities

  • Triage medical information in a specific medical or surgical specialty
  • Schedule post-surgery diagnostic procedures and follow-up patient appointments
  • Act as a liaison between patients, consulting physician, referring physician, or outside agency to coordinate patient appointments, admission, or surgery
  • Coordinate the exchange of information either written or verbally before and after patients are seen
  • Coordinate anesthesia
  • Obtain all necessary medical records, diagnostic studies, pathology items, etc., that are needed for patient care
  • Assist in scheduling new patient appointments by triaging to appropriate department and physician
  • Enter appointment information in online appointment system
  • Coordinate pre-admission tests and procedures
  • Schedule and ensure pre-anesthesia works-ups are completed
  • Coordinate the direct transfer of patients from outside hospitals to UWMC with physicians, medical staff, and Admitting
  • Other assigned duties
