About The Position
Pride Health is hiring a Patient Care Coordinator to support our client’s healthcare facility in New Port Richey, FL 34654. This is a 3-month contract assignment with the possibility of extension or conversion, offering a great opportunity to work with a top-tier healthcare organization.
Requirements
- High School Diploma, GED or equivalent.
- Previous referral experience a plus.
Responsibilities
- Process and manage incoming and outgoing patient referrals.
- Coordinate with specialists and imaging centers to obtain consultation notes and medical records.
- Answer and route incoming phone calls professionally.
- Follow up on pending referrals and documentation.
- Maintain accurate patient records and referral information in the EMR.
- Communicate with providers, patients, and external healthcare offices regarding referral status.
- Ensure referrals are completed in a timely manner.
- Provide general administrative support to the clinical team.
