Requirements

• High school diploma or GED required
• Strong communication and customer service skills
• Comfortable using computers, email, and electronic medical records
• Ability to manage multiple priorities while maintaining attention to detail
• A proactive, team-oriented mindset with a passion for helping others

Nice To Haves

• Healthcare experience desired. We welcome individuals who have worked in healthcare, senior living, medical offices, customer service, or other service-focused environments.

Responsibilities

• Coordinating patient care and follow-up services
• Managing incoming calls, emails, and patient inquiries
• Processing provider orders, including medications, labs, imaging, referrals, and home health services
• Communicating care plan updates with patients, families, community staff, and healthcare partners
• Supporting medication reconciliation and escalating concerns to providers when needed
• Assisting with post-discharge outreach and ensuring appropriate follow-up care is scheduled
• Identifying urgent patient concerns and helping coordinate timely interventions
• Maintaining accurate documentation and patient records
• Supporting telehealth appointments and care coordination activities
