About The Position

The Patient Care Coordinator plays an essential role in supporting both the patient experience and the day-to-day administrative operations of the practice. This individual serves as a primary point of contact for patients and is responsible for ensuring timely communication, coordination of care, and accurate administrative support throughout the patient journey. The Patient Care Coordinator works collaboratively with clinical and administrative teams to promote continuity of care, operational efficiency, and an overall positive patient experience.

Requirements

  • High school diploma or equivalent required
  • Proficiency in Microsoft Word, Excel, Google Suite, and Electronic Health Record (EHR) systems.
  • Strong organizational skills with the ability to prioritize tasks and manage multiple responsibilities simultaneously.
  • Excellent verbal and written communication skills with a professional and compassionate demeanor.
  • Strong attention to detail and ability to maintain accurate documentation and records.
  • Ability to work collaboratively within a team-oriented healthcare environment.
  • Demonstrated punctuality, reliability, and professionalism.
  • Ability to remain calm, empathetic, and solution-oriented when supporting patients and resolving concerns.

Nice To Haves

  • Associate’s or Bachelor’s degree preferred.
  • Previous experience in healthcare administration, medical reception, patient coordination, customer service, or a related field preferred.
  • Experience working in behavioral health, outpatient healthcare, or a fast-paced medical environment is a plus.

Responsibilities

  • Schedule appointments and address patient inquiries via phone, email, text, and patient communication platforms.
  • Handle incoming appointment requests and initiate timely follow-up communications with patients and providers.
  • Provide administrative support related to patient care needs, including prescription refill requests, prior authorizations, referrals, and medical record transfers.
  • Assist patients experiencing telehealth or technological challenges and provide guidance on accessing virtual appointments and patient portals.
  • Maintain accurate and up-to-date patient records within the Electronic Health Record (EHR) system.
  • Verify insurance information, collect required documentation, and support insurance claim submission processes.
  • Monitor online referral platforms, voicemails, and incoming faxes to ensure timely follow-up and coordination of care.
  • Process invoices, submit claims, and support billing-related administrative tasks as needed.
  • Coordinate communication between patients, providers, and internal departments to support continuity of care and efficient scheduling workflows.
  • Escalate patient concerns, scheduling conflicts, or urgent matters to the appropriate clinical or administrative leadership when necessary.
  • Maintain professionalism, confidentiality, and compliance with HIPAA and organizational policies at all times.
  • Contribute to a welcoming, patient-centered environment that reflects PAX Health’s mission and values.
  • Abide by PAX Health’s Code of Ethics and model organizational values in all interactions and responsibilities.
  • Perform additional administrative or operational duties as needed within the scope of the role.
