About The Position

The Patient Care Coordinator (PCC) serves as a key member of the Pre-Anesthesia Evaluation Clinic team, providing administrative and scheduling support to ensure patients are prepared for surgery. Responsibilities include coordinating pre-anesthesia appointments, managing high-volume scheduling and incoming calls, reviewing referrals, obtaining and verifying patient demographic and insurance information, and facilitating communication between patients, providers, surgical clinics, and ancillary departments. The PCC supports efficient clinic operations by maintaining accurate documentation in Epic, assisting with care coordination activities, monitoring work queues, and helping ensure patients complete required pre-operative evaluations within designated timelines. This position is fully onsite, Monday through Friday, and plays a critical role in delivering an exceptional patient experience while supporting safe and timely surgical care. This is a Stanford Health Care Tri-Valley job.

Requirements

  • High School Diploma or GED Required
  • 3+ years to 5 years medical office experience Required
  • Excellent communication skills.
  • Excellent phone and computer skills.
  • Working knowledge of medical terminology.
  • Working knowledge of insurance terminology.
  • Strong organization skills and attention to detail.

Responsibilities

  • Greets patients and manages phone calls
  • Provides information on various department clinical programs to patients during patient scheduling/intake call
  • Schedules new and returning patient appointments
  • Coordinates virtual classes and support groups
  • Downloads and prints patient glucometer and continuous glucose monitor data
  • Assists with getting patient weights
  • Creates patient education packets for various clinical programs both inpatient and outpatient
  • Manages referral que and generates referrals when appropriate
  • Sends communication to referring physician offices when appropriate
  • Scans patient related documents for various clinical programs
  • Requests relevant laboratory data in preparation for patient visits
  • Assists the bariatric patient care advocate with guiding patients through the bariatric program
  • Builds customer relations through positive interaction with patients and other hospital departments
  • Consistently and accurately registers patients in Epic with all required information and keeps up with new registration requirements
  • Responsible for obtaining insurance and/or employer authorization as applicable.
  • Responsible for explaining benefits and co-payments to patients as applicable.
  • Responsible for subpoena and medical record requests, locating medical reports in various systems.
  • Ensures the daily review of email system for department communications.
  • Participates in department in-service programs.
  • Performs education modules monthly and HealthStream education when due.
  • Assists with inventory and replacement of supplies by ordering or notifying supervisor.
  • Attends huddles and mandatory in-service meetings.
  • Accepts assignment of other duties as needed, receives training and/or orientation.
