About The Position

The Patient Care Coordinator is the first point of contact for patients accessing services through Resolve Sleep Enhanced Services. In this important front-line role, you'll be the first point of contact for patients—setting the tone for a positive, supportive experience from start to finish. You’ll help keep the clinic running smoothly, support our clinical team, and play a key role in delivering excellent care.

Requirements

  • A passion for people and exceptional customer service skills
  • Detail-oriented and a quick learner who thrives in a busy clinic
  • A team-first mindset and a solutions-focused approach
  • Previous experience in healthcare, administration, or customer service
  • Comfort with digital tools, including attention to detail and accuracy in EMR systems, documentation and virtual care (training provided)

Nice To Haves

  • Bilingual French and English preferred.

Responsibilities

  • Guiding patients through their care journey, including scheduling appointments with Nurse Practitioners and ensuring all required forms and documentation are completed prior to appointments
  • Coordinating referrals and working closely with clinics that refer patients for CPAP prescription support, Home Sleep Apnea Testing (HSAT), Oral Appliance Therapy (OAT), supplements, CBT-I, and other Enhanced Services programs
  • Facilitating home sleep testing (where applicable), including patient communication, education, scheduling, shipping coordination, and follow-up
  • Supporting web-generated referrals and helping patients access services such as CBT-I, supplements, sleep consultations, and other virtual care programs
  • Identifying opportunities for patients to benefit from additional sleep health services and programs, and helping connect them with the most appropriate care pathway
  • Monitoring patient progress through established workflows and ensuring timely follow-up, appointment completion, and communication between patients, clinicians, Nurse Practitioners, and partner clinics
  • Maintaining accurate documentation and administrative records within company systems
  • Acting as a role model and ambassador to the company by living our shared Resolve Sleep Health values
