About The Position
This role focuses on the coordination of care transitions and care management to promote enhanced quality of care within physician practices.
Requirements
- Graduation from an accredited nursing program with an Associate's degree in Nursing (ASN).
- 2 years of clinical experience in nursing.
- Registered Nurse Licensure from the state of Indiana
- CPR required.
Nice To Haves
- Graduation from an accredited nursing program with a BSN or MSN in nursing science or related field.
- 3 - 4 years of clinical experience in nursing showing leadership skills in field of interest.
