As a Patient Care Coordinator (LVN) , you will serve as a primary point of contact for patients throughout their care navigation journey. You'll help patients understand and access their surgical benefits, coordinate communication across stakeholders, and ensure a seamless experience from enrollment through care coordination. This role is ideal for someone who enjoys patient engagement, thrives in a fast-paced environment, embraces new technology, and is excited about helping improve healthcare delivery.

Requirements

  • Active Licensed Vocational Nurse (LVN) license in any U.S. state
  • 2–7 years of experience in healthcare care coordination, patient engagement, case management, or a related field
  • At least 2 years of remote healthcare experience
  • Experience working with patients via phone, including outreach and engagement activities
  • Strong communication, organizational, and relationship-building skills
  • Comfort working in a fast-paced, evolving environment
  • Experience using digital tools such as Google Workspace, Salesforce, EMRs, or healthcare technology platforms
  • Interest in leveraging AI and emerging technologies to improve workflows and patient experiences
  • Ability to work independently while collaborating effectively across teams
  • A patient-first mindset and commitment to delivering exceptional service
  • Strong attention to detail and follow-through
  • Adaptability and curiosity in a technology-driven environment
  • Confidence navigating ambiguity and contributing to process improvements
  • Enthusiasm for working as part of a collaborative, high-performing team

Nice To Haves

  • Experience in healthcare technology, digital health, or startup environments
  • Familiarity with surgical care pathways, including orthopedic, spine, bariatric, or cardiac procedures
  • Bilingual proficiency in Spanish or Hindi
  • Experience supporting self-insured employer healthcare programs or value-based care models

Responsibilities

  • Conduct proactive outreach to eligible patients and educate them about available surgical care benefits
  • Guide patients through enrollment, onboarding, and eligibility verification processes
  • Collect and organize required medical records and documentation
  • Coordinate communication among patients, providers, nurses, and internal operations teams
  • Maintain accurate patient records and case documentation within company systems
  • Provide exceptional customer service and support throughout the patient journey
  • Share patient insights to help improve workflows, processes, and technology solutions
  • Collaborate with leadership and cross-functional teams to support operational excellence and growth initiatives
